As the instance administrator, I want to complete a first-install setup, so that I get an admin account without relying on insecure default credentials

Description
On first install, before any account exists, the administrator is guided through a setup screen to create the first account. This is the only way to create an account without being invited or already having an admin session — there is no self-service sign-up.
Once the first account is created, the setup screen is no longer accessible.
